anti-metastasis effects of both interferon-

The 1996-2000 average annual age-adjusted incidence rate for invasive cancers of the liver and intrahepatic bile duct was almost
three times higher in males than in females (6.8 compared to 2.4 per 100,000), and blacks of both sexes had higher rates compared
to whites.
There were 708 cases of liver /intrahepatic bile duct cancers diagnosed and reported among residents of Pennsylvania for 2000
– the highest annual figure recorded between 1991 and 2000. The annual age-adjusted incidence rates by sex and race for ten years
(1991 through 2000) showed increasing rates among males, especially black males, and females of both races.
The 1996-2000 average annual age-specific incidence rates show that black males have had the highest risks. During the period
of 1996-2000, over 62 percent of the cases were diagnosed among residents 65 years of age or older. In 2000, over 57 percent

Following the discovery of hepatitis B virus
in 1964, hepatitis viruses A and C were identi-
fied in 1972 and 1989, respectively. The roles of
these viruses in Japan have been increasingly
well clarified. That is, the pathophysiology of
the three types of viral hepatitis has been eluci-
dated, and appropriate treatment measures
have been established.
In the past, it was a clinical practice at out-

Revolutionary liver cancer treatment to be showcased at Medway
The Medway NHS Trust is hosting a ground breaking clinical workshop on 17 February
2004 at the Medway Maritime Hospital, Gillingham which will include two live demonstrations
of an exciting new procedure for the diagnosis and treatment of liver cancer . The
internationally respected specialist, Professor R Lencioni of Pisa Hospital, Italy, will perform
this procedure, which is based on Contrast Enhanced Ultrasound (CEUS) and

Overview
A large number of malignant lesions have been described in the liver , both of primary hepatic
origin, and metastatic in nature (Table 1). The
most common primary hepatic malignancy is
hepatocellular carcinoma, accounting for over
85% of primary cancers of the liver , while
intrahepatic cholangiocarcinoma is the next
most common disease, constituting up to 10%
of reported cases. Metastatic disease to the
liver is considerably more common than
primary hepatic malignancy.

• Intra-arterial irradiation has been developed in the treatment of inoperable liver
tumours. This is to eradicate tumour cells by radiation.
• Radiologist with special training in Interventional Radiology will perform this
treatment in the Department of Radiology under image guidance.
• The procedure involves the selective cannulation of the specific artery supplying the
liver tumours and the subsequent delivery of radioactive particles (Yttrium-90).

This chapter is comprised of three sections. The first section reviews
how factors that alter liver metabolism can cause toxicity from anti-
cancer drugs. Because of the high incidence of liver toxicity and the high
case-fatality rate, the second section will be an in-depth examination of
liver toxicity in hematopoietic stem cell transplantation. The final sec-
tion will review hepatotoxicity from individual anticancer drugs.
ALTERATIONS IN HEPATIC METABOLISM OF DRUGS
Chemotherapeutic agents usually have a narrow therapeutic index.
Thus, factors that change disposition of these drugs may easily alter
efficacy or cause toxicity. Specific recommendations about dose reduc-
tion in patients with impairment or alteration of hepatic metabolism
will be discussed in other chapters. This section will discuss the impact
