UPERIOR sulcus or Pancoast tumors are primary lung tumors invading both the chest wall and vertebral column and may be characterized by pain, Horner syndrome, and extention into the bone. Conventionally, Stage IIIb superior sulcus tumors invading the vertebral column have been considered unresectable, and the thera- pies of choice have been radiotherapy alone or radiothera- py followed by resection. Recent advances in spinal in- strumentation and wider acceptance of total spondylectomy, however, make more radical surgical modalities feasible. Specifically, we have devised a new surgical technique allowing for the practical en bloc resection of primary lung cancer invading the chest wall and spinal column.
