It is a disease that starts in the marrow.
The marrow – the spongy tissue found in the center of the bone – is the
part of the body that makes red and white blood cells.
In myeloma , one type of developing white blood cell (B lymphocyte),
called a plasma cell, grows continuously to form a mass, or tumor, in
the marrow.
• When there are too many plasma cells, they crowd out developing,
normal white and red blood cells.
• The plasma cells in myeloma make factors that cause bone to
dissolve, leading to fractures.
• Fewer red blood cells result in less oxygen being carried to the tissue
throughout the body, causing fatigue and shortness of breath with
exercise.
• Fewer white blood cells make it harder to fight infection.

The Myeloma Institute for Research and
Therapy launched the Patient Initiated
Quality Improvement Project in January.
The pilot program is designed to improve
the quality of clinical care and customer
service at the center.
The six-month project, funded by a grant
from the Cynthia and George Mitchell
Foundation, involves the analysis of
detailed patient feedback by a committee
comprised of University of Arkansas for
Medical Sciences staff and Myeloma
Institute patients. A total of twelve patients
are providing the committee with elec-
tronic feedback regarding their clinical
experiences and other aspects of treatment
via e-mail, according to Dr. Elias Anaissie,
the Myeloma Institute’s director of the

me. But I’m very good at one thing: I can absorb an enormous amount of data, condense it into a summary,
and communicate the results of that data to other people in such a way that they will understand it. That’s
what I did in business and that’s what I’ve been doing with information about myeloma . Mostly, I do this

Compound from marine bacteria shows potential as multiple myeloma therapy
Kills blood cancer cells with low toxicity in preclinical studies
BOSTON—An anti-cancer compound derived from bacteria dwelling in ocean-bottom
sediments appears in laboratory tests to be a potent killer of drug-resistant multiple myeloma
cells, and potentially with less toxicity than current treatments, report Dana-Farber Cancer
Institute researchers in the November issue of Cancer Cell .
The experimental compound, NPI-0052, has been found to block or inhibit cancer cells’
proteasomes from working effectively. The proteasome work as a cell’s “garbage disposal,”
chewing up and disposing of old, unwanted proteins. With their proteasome jammed, cells die
from the backup of damaged proteins.

8 years old male presented with the history of
swelling of feet, puffiness of face and diminished
urine output following strenuous road journey in hot and
humid weather and consumption of 6 tablets of voveran
for backache. Investigation revealed raised ESR (155
mm) and anemia (Hb 7.5gms%). Bence-Jones
proteinuria and lytic bone lesions were not detected.
Blood urea and serum creatinine were 210 and 12.5
mg% respectively. Serum calcium was 7.5 mg%.
Immunofixation electrophoresis revealed positive band
in IgA and kappa region.
With a diagnosis of IgA multiple myeloma with acute
renal failure, he was started on twice weekly
hemodialysis and monthly pulse chemotherapy
consisting of melphalan and prednisolone. A kidney

This policy addresses high-dose chemotherapy with hematopoietic s tem-cell support as a treatment of mul-
tiple myeloma . Bone marrow transplants typically include high-dose chemotherapy (HDC).

Rate Trends
Myeloma , also known as multiple myeloma
or plasma cell myeloma , is the second most
common blood cancer in the United States
and comprises approximately 1 percent of all
cancers. Unfortunately, there has been little
change in the overall incidence rate of myeloma ,
and the mortality rate has increased over the past
three decades. The incidence rate is higher in
men than women. Myeloma is more common
among the elderly, and African Americans
have approximately double the incidence and
mortality rates of Whites.

ultiple myeloma (MM) is 1 of the 3 clinical entities in
which plasma cell dyscrasias may appear in the head
and neck. The other 2 forms are represented by extramed-
ullary (EMP) and solitary bone plasmacytomas (SBP). Such
types of plasma cell lesions are histologically indistinguish-
able from each other, thus representing distinct manifesta-
tions of a continuous pathologic spectrum.

appropriate differential diagnosis rests solely on clinical

ultiple myeloma , sometimes called
plasma cell myeloma , arises in bone marrow
and involves the plasma cells, a type of
blood cell that normally plays a crucial role
in the immune response by producing
antibodies. It is classified as a lymphoproliferative disease
and is therefore related to the lymphomas. In some early
classification schemes, multiple myelomas were included in
statistics on lymphomas.
There is considerable variation in the
incidence of the disease. Among men, age-
adjusted incidence rates (per 100,000) range
from a low of 1.6 among Japanese to a high
of 11.3 among blacks, a seven-fold
difference. This difference is particularly
interesting since the opposite relationship
holds for Ewing’s sarcoma, a type of bone
