IBMS Sussex Branch: Multiple Myeloma Dr Graham Meade recently spoke to the Sussex Branch on the topic of multiple myeloma . He explained that the disease was a tumour of plasma cells. The proliferating cells were monoclonal and tended to form multiple foci in the bone marrow. As the cells were monoclonal they all secreted an antibody that was identical in both type and specificity. In 83% of cases an intact antibody was produced. Free light chains only were secreted in 15% of patients. In the remaining 2% there was no evidence of secretion of immunoglobulin or its components. A progressive spectrum of disease was observed. The mildest form was called monoclonal gammopathy of uncertain significance (MGUS). This stage could persist for a number of years and was best left untreated with the patient being monitored for signs of disease progression. Miura and Ken-ichi Sawada We studied 48 patients with multiple myeloma and its related disorders (plasma cell dyscrasias: PCD). Clonal changes were observed in 19 patients (39.6%), which included 9 untreated and 10 treated patients. Chromosomal gains of 3, 11 and loss of 13 were the most frequent numerical chromosome aberrations. Chromosome 13 was lost at an early stage and 17p (p53) abnormality appeared in an advanced stage of the disease. The most common additional region was 1q which contained a locus of the IL-6 receptor gene instead of 7p,the locus of the IL-6 gene. Virtually all patients with an elevated plasma cell labelling index or circulating plasma cells in the peripheral blood have active multiple myeloma . The physician must realise that the presence of a normal plasma cell label- ling index or absence of circulating plasma cells does not exclude the possibility of active multiple myeloma . The second most common hematologic malignancy, this diagnosis is newly made in 35,000 persons yearly worldwide. Over 40 years ago, Bergsagel and others showed that a combina- tion of melphalan and prednisone was an effective chemotherapy reg- imen in myeloma , inducing responses in over 50% of newly diagnosed patients. Thalidomide and Its Derivatives: New Promise for Multiple Myeloma Donna Weber, MD Background: The combination of melphalan and prednisone has been accepted as standard treatment for multiple myeloma (MM) because most studies demonstrate only minimal survival benefit of combination chemotherapy regimens when compared with melphalan and prednisone. Despite modest gains with more intensive myeloablative regimens for certain subgroups, myeloma remains incurable. In 1999, investigators at the University of Arkansas reported the promising results of a phase II study of thalidomide in patients with resistant MM. Since then various trials of thalidomide alone, and in combination, have been tested in patients with resistant, and more recently, untreated MM. In addition, preliminary results of phase I studies of the immunomodulatory derivatives (IMiDs) of thalidomide have recently been reported. ...
