our understanding of prostate can-
cer: refinements in molecular and
cell biology technology, better
diagnostic and staging methods,
improvements in local treatments,
and the discovery of more effec-
tive chemotherapies, just to men-
tion a few. Still, there is much to
learn about this common disease.
We lack knowledge of the funda-
mental mechanisms leading to the
initiation, promotion, and progres-
sion of this cancer . We have multi-
ple choices, but we still do not

PROSTATE CANCER : Surgery and radiation cause different complications.
Researchers led by a team from the National Cancer Institute compared the side effects of
treatments for localized prostate cancer in 1,591 men between the ages of 55 and 74. Two years
after treatment, the 1,156 men who had surgery had more problems with urinary incontinence
(9.6 percent vs. 3.5 percent) and impotence (79.6 percent vs. 61.5 percent) compared to those
who had radiation. The 435 men who had radiation had more bowel problems such as diarrhea
(37.2 percent vs. 20.9 percent). Ninety-two percent of all patients said they would choose the
same treatment again.
This is the first study to directly compare side effects of radiation vs. surgery for the treatment of
localized prostate cancer in a diverse group of patients.
The men were surveyed about side effects at six, 12 and 24 months after diagnosis. The

• Prostate cancer is a problem. There is screening but no money for follow up.
• Prostate screenings are being funded through the tobacco settlement funding and are
not connect to what a person may owe the system. No one should be refused as long
as the money is there.
• Johns Hopkins Hospital will not do a screen for prostate cancer if person owes money
to their system.
• There are eligibility criteria but because of the high rate of prostate cancer in African
American men, we will screen any African American man over 45. If they have no
insurance, there is no cost. If they have insurance, the funding pays for what ever the

n the past, if someone mentioned being
screened for prostate cancer , Frank
Milat’s response was the same as a lot of
men: “What I don’t know can’t hurt me.”
After undergoing a free screening at UAMS
Medical Center last September, Milat knows
differently. From the results of his screening,
he became one of more than 209,000
American men who were diagnosed with
prostate cancer in 1998. Under the care of
UAMS Medical Center urologist Graham
Greene, M.D., Milat has received successful
treatment over the past several months.
However, he never forgets the importance of
that initial screening.

controlled trials (RCTs) are the best
studies on which to base such recommendations. In the
absence of RCTs, researchers and policymakers often
examine less desirable cohort studies with concurrent
nonrandomized controls and case-control studies. Un-
fortunately, in the area of early detection and treatment
of prostate cancer , little controlled data are available, re-
gardless of study design. A single case-control study has
shown no evidence of benefit from digital rectal ex-
amination (DRE), in terms of lower exposure odds to
