Tularemia with peritonitis developed in a 50-year-old
man soon after diagnosis of stomach cancer with metasta-
sis. The ascites grew Francisella tularensis subsp. holarc-
tica , which was identified by sequencing analysis of the
16S rDNA. The infection resolved with antimicrobial treat-
ment. Antibodies detected 4 weeks after onset disap-
peared after chemotherapy-associated lymphopenia.
Case Study
A 50-year-old man arrived at the emergency depart-
ment in September 2003 with a 2-day history of high fever
(temperature up to 40.8°C), rigors, dry cough, nausea,
vomiting, lower abdominal pain, and melena. The patient
had recently been diagnosed with signet-ring–cell carcino-
ma of the stomach with evidence of metastasis to the lung

cer are found in Poland. Reasons for the increased incidence
are not known, but high intake of sausages and other pre-
served foods and low intake of fresh fruits and vegetables

controls randomly selected from the general population was

INTRODUCTION
Bile acids are polar derivatives of cholesterol essential for
the absorption of dietary lipids, and regulate the transcription
of genes that control cholesterol homeostasis. Depending on the
nature of their chemical structure, different bile acids exhibit
distinct biological effects (1). After synthesis by the liver, and
their excretion into the bile canaliculus and digestive tract, the
primary bile acids, cholic acid (CA) and chenodeoxycholic acid
(CDCA), are metabolized by enteric bacteria to produce secon-
dary bile acids, primarily deoxycholic acid (DCA), ursodeoxy-
cholic acid (UDCA) and lithocholic acid (LCA). Bile acids are
conjugated to glycine or taurine when the glycine conjugates
predominate. Conjugation of bile acids to glycine and taurine
is one mechanism by which an organism can decrease the

INTRODUCTION
n most countries cancer incidence and mortality rates are
constantly increasing, and according to available predic-
tions this tendency will persist in the nearest future. At the
Congress of the International Union Against Cancer in Rio de
Janeiro in 1998, a progressive increase in the number of incident
cancer cases in the world was noted: 6 million cancer cases in
1975, 7.6 million in 1985, 8.1 million in 1990, and the expected
rate in 2010 is 13 million incident cancer cases. The fact that this
increase is taking place on the background of doubtless achieve-
ments of modern oncology in diagnostics and treatment of tumors
provides reason enough to primary and secondary cancer pre-
vention to be the main means of changing this tendency.

Abstract. CDX2, a transcriptional factor expressed in the
intestine, is implicated in the development and maintenance
of the intestinal mucosa. Recent studies have demonstrated
that CDX2 is expressed in the intestinal metaplasia of the
stomach and intestinal-type gastric cancer , while it is not
expressed in the normal gastric mucosa. To investigate the role
of CDX2 in gastric cancer , we determined CDX2 expression
and cell proliferation rate in various types of gastric cancer
tissues by immunostaining. Surgically dissected gastric cancer
tissues were collected from 40 patients. Consistent with
previous reports, CDX2 was expressed in most gastric mucosa
samples with intestinal metaplasia (89%, 16/18), although it
was not found in the adjacent normal mucosa. CDX2
expression was also detected in 64% (18/28) of intestinal-
type gastric cancer cases, whereas it was not observed in the

Summary
Relative survival after a diagnosis of cancer of the stomach was poor when compared with
other cancer sites. For the period 1992–1997, relative survival one year after diagnosis of
cancer of the stomach was 46.4% for males and 47.0% for females. The five-year relative
survival proportion for males was 22.6% and for females was 24.8% (Table 4.1). Survival ten
years after diagnosis was 20.1% for males and 21.4% for females in 1987–1991, the most
recent period for which ten-year relative survival data are available (Tables 4.2 and 4.3).
Between the diagnosis periods 1982–1986, 1987–1991 and 1992–1997, stomach cancer
relative survival increased. This increase was significant between 1982–1986 and 1992–1997

The 1994-98 average annual age-adjusted incidence rate for stomach cancer was approximately 144 percent higher among males,
compared to females, (10.0 versus 4.1, respectively). Black males had a much higher average annual age-adjusted incidence rate
(16.7), compared to the other three sex/race groups. The lowest average annual age-adjusted incidence rate (3.7) was recorded
for white females.
